INSTRUCTIONS FOR

NEW MEMBER APPLICATION FORM

  1. Do not write in the first 4 lines of the form unless you know the name of your proposer
  2. Start the application on line 5 with your name
  3. On page 2 of the application form, fill in number 11(B) (Previous Places of Residence), if applicable
  4. Submit the completed application with a $50 application fee to the office
